About ChartDraft AI
ChartDraft AI is an ambient clinical documentation assistant that captures outpatient visits and drafts clinician-approved notes and billable artifacts — so providers can focus on the patient in front of them.
Our Story
ChartDraft AI was founded on a straightforward observation: clinicians spend too much of their day documenting care instead of delivering it. After-hours charting has become the norm, not the exception. The result is documentation burnout, strained provider-patient relationships, and a rising rate of coding errors that affect reimbursement and compliance.
We set out to change that — not by replacing clinical judgment, but by handling the mechanical burden of documentation. ChartDraft AI listens during the encounter, drafts structured notes and billable artifacts, and presents them for clinician review. The provider approves, edits, or rejects. Nothing reaches the chart without their explicit sign-off.
The approach is deliberate: ambient AI that drafts, clinicians that approve. We believe documentation tools should support the clinician's authority, not circumvent it.
Our Mission
“To give clinicians back the time documentation takes away, without sacrificing accuracy, compliance, or control.”
Our Values
Every artifact is evidence-oriented and structurally aligned with documentation standards. We treat accuracy as a prerequisite, not a feature.
Low-friction workflows that integrate quietly into the clinical day. No alarms, no interruptions — just reliable support in the background.
Every statement in a draft traces back to a specific moment in the encounter. Clinicians can verify any claim before it reaches the chart.
Drafts are previews, never outcomes. Nothing posts to the EHR without explicit clinician review and approval.
How We’re Different
- Closed-loop approval — no note is finalized or posted without clinician sign-off
- Per-sentence provenance — every line links to the source moment in the encounter
- EHR-grounded retrieval — drafts reference the patient's existing record for context and continuity
- Downstream-ready outputs — orders, referrals, and coding suggestions drafted alongside the note
- Integration-minded architecture — built on SMART on FHIR and HL7 for reliable EHR interoperability
